Boat Tour Waiver

This needs to be filled out by everyone (except for Kristal & Owen), even if you plan to go on the boat but not snorkel. Adults need to complete it again for their children. 

​

We have also put together a survey to assist the boat crew with preparations. Let us know if you'll be snorkeling and, if so, your shoe size here.

 

Mango Farm Waiver

This is required for: Anne, John B, Amy, Matt, Macie, Matty, Lauren & Ruby

 

Chocolate Farm Waiver

​

This is required for: Ashley, Landon, Rachel, Karl, Vivian, Margaret, Jim & Chris. Adults will sign for minors. 

​

Zipline Waiver

 

This is required for: Zach, Nicole, Harland, Bennett, & Jameson.

*The ATV tour will require waivers, but they are not yet available for our group. If they become available ahead of time, Ashley will update you, otherwise plan to sign on location*

 

ATV waivers will be needed for Casey, Brady, Chloe, Fred, Kristal, Will & Owen.
